Request Your Medical Records

Our Release of Information staff will be happy to assist you with requests for your medical records. We also assist providers with completing forms for patients.

We can assist you with the following —

  • access to your medical records
  • obtaining copies of your medical records
  • requests to amend your medical records
  • completion of forms for benefits, insurance, and other reasons

Release of Information staff are experts in our patients' rights and their medical records.

Information about your medical care can be released to your private doctor, employer, insurance company, social service agency, etc. upon your request.  Ask clerks in admitting or outpatient clinics to help you complete and sign the release of information form.  Be sure to ask your doctor for a return to work statement.

How to Request Information

By Mail:
To request a medical record, please one of the forms listed in the column to the right, complete the requested information, sign the form, and mail it to the following address —

By Fax:
Fax your signed form to 612-725-1340

Self-Service Kiosks:
You can request your medical records through the self-service kiosks at each site by selecting "Request Medical Records" from the Welcome menu.

Emailed forms cannot be accepted.

Requests for records may take up to 20 business days to process.


There is no cost to send copies directly to another health care provider. If copies are for a patient's personal use, photocopying fees may be assessed
