Application requirements:
  • Pharmacy School academic transcripts - Minimum GPA 3.0 during didactic coursework; no repeated courses for pass/fail curriculum
  • Curriculum vitae
  • Letter of intent
Please address the following: 
  • Why do you want to complete your residency at the Grand Island VA
  • Areas of clinical interest
  • Strengths and what you are hoping to accomplish during residency
  • Short and long-term career goals
  • Why you are a good match with the Grand Island VA
  • Three (3) completed residency applicant recommendation request forms (from PhORCAS*) - please have reference writers complete and answer all narrative comment questions (1-4) on the standardized reference form
  • PGY2 candidates:
    • Must complete at least one primary care/ambulatory care rotation during PGY1 year
    • Supplemental Application Material: PDF of PharmAcademic Evaluation for primary care/ambulatory care rotation completed during PGY1
    • PGY2 candidates must have reference from the PGY1 RPD
Application deadline: January 10
    Interview notification: January 27

    Appointment is contingent upon satisfactory completion of a physical exam, including negative drug test.  Applicants must meet requirements for federal employment.  Residents are required to obtain a pharmacist’s license in any state by October 1st, however it is strongly encouraged that residents are licensed by program start date.
