The Grand Island facility includes a Nursing Home Care Unit, Residential Substance Abuse Unit and a Community Based Outpatient Clinic.

The Grand Island Nursing Home Care Unit is a 65-bed facility.  Services provided by the Nursing Home Care Unit include extended care, rehabilitation, geriatric care, palliative care, respite care, supportive/restorative and long-term care, and general nursing home care.

  Follow US 281 South.  At Airport Road, continue heading straight.  It becomes North Broadwell Avenue.

  Follow US 281 North.  Turn right onto Capital Avenue (1.2 miles).  Turn right onto North Broadwell Avenue.

  Take I 80 West.  At exit 312, take US 34/US 281 exit toward Grand Island.  Turn onto US 281 North/US 34 East.  Follow US 281 North (8.8 miles).  Turn right onto Capital Avenue (1.2 miles).  Turn right onto North Broadwell Avenue.
• From the West
  Take I 80 East.  At exit 312, take US 34/US 281 exit toward Grand Island.  Turn onto US 281 North/US 34 East.  Follow US 281 North (8.8 miles).  Turn  right onto Capital Avenue (1.2 miles).  Turn right onto North Broadwell Avenue.
